Forms Related to PON 1098

The Request for Project Extension (pdf) form is to be completed by an installer when there is an anticipated delay in the delivery of equipment to the customer. To complete the form, the following three items are required:

• Explanation for delay in delivery of equipment

PON 1098 SEQRA Issues Outline

The SEQRA Issues Outline provides an overview of the State Environmental Quality Review Act and its requirements as they pertain to permitting small wind turbines under PON 1098.
